Tivi Way — Workshop & Event Participation Waiver
In consideration of my child(ren) and/or myself participating in workshops, classes, or events at Tivi Way (Tivi Tray LLC), 55 Liberty Rd, Suite A109, Sykesville, MD 21784, I acknowledge and agree to the following:
1. Assumption of Risk. I understand that participation in hands-on creative activities involves inherent risks, including but not limited to the use of craft materials, tools, and equipment. I voluntarily assume all risks associated with participation.
2. Supervision. For caregiver-accompanied programs, I agree to actively supervise my child(ren) at all times while on the premises. For drop-off programs, I confirm my child meets the stated age requirements and I have disclosed any relevant allergies or medical needs at booking.
3. Release of Liability. To the fullest extent permitted by Maryland law, I release and hold harmless Tivi Tray LLC, its owners, employees, and agents from any claims arising out of ordinary negligence in connection with participation.
4. Medical Authorization. In the event of an emergency, I authorize Tivi Way staff to seek medical treatment for my child if I cannot be reached.
5. Photo Release. I understand Tivi Way may photograph activities for promotional use; I may opt out by notifying staff in writing.
This waiver applies to the workshop(s) and event(s) in this order. A parent or legal guardian must sign for participants under 18.
